This course is designed to teach the attendees how to use process driven root cause analysis, and includes some of the most popular and useful problem solving techniques available. These objectives are accomplished with a simple set of learning aids that can be used each time Root Cause Analysis (RCA) is applied in your facility. Methods that will be covered include:
Change Analysis;
5 Whys;
Fault Tree;
Logic Tree;
Sequence of Events;
Event and Causal l Factor Ana l ysis;
Simplified Failure Modes Analysis (FMA) ; and
Failure Modes, Effects, and Critica li ty Ana l ysis (FMECA).
Class lecture will be reinforced with numerous practice opportunities, so the attendee wi ll be prepared to go back to the job and attack chronic plant problems and incidents right away. During the in-c l ass time, you will l earn to :
Outline the steps of a successfu l root cause process;
App l y tools like Failure Reporting, Ana l ysis, and Corrective Action System (FRACAS) to identify reoccurring issues;
Define which events shou l d trigger a root cause investigation;
Describe when to use each RCA tool;
Build business case thinking into every step of your RCA process
Recall how to ensure success and sustainab l e so l utions.
